Custom Order Form

Please submit the following order form to request a design spot. You will be contacted within 3 business days to confirm your reservation. Once a date is confirmed, you have 5 business days to submit a non-refundable deposit of $100.00 per costume to secure your spot.

Terms and Conditions

  • Designs are original creations for each client and I do not reproduce other designer’s or client’s creations.
  • Reservations that are not confirmed by a deposit within 5 business days may forfeit their design spot.
  • Each costume order includes 30 minutes of client consultation.
  • There are no refunds or exchanges on any items.
  • Any concerns or issues must be expressed within 7 days of receiving the costume.
  • Changes requested after a design is approved and/or constructed are subject to a change fee.
  • Shipping is only available within Canada. Clients may select Canada Post Express Post Shipping or Purolator. Shipping fees are an additional charge.
